COMMON PLEAS COURT
WHEREAS, Obligor has been ordered by this Court to pay child support, alimony or spousal support; NOW, THEREFORE, IT IS ORDERED that OBLIGOR _________________ born_______________D.O.B., Soc. Sec. No. ____________________ seek employment if able to engage in employment by making three (3) job applications per week at three (3) different places of employment and providing written proof of the same to the Washington County Child Support Enforcement Agency (WCCSEA). Obligor shall obtain from the WCCSEA blank job application forms and each week shall return three (3) completed forms signed by three different prospective employers. Any additional instructions will be given to the Obligor by the WCCSEA when Obligor picks up the blank forms. Obligor's first set of employment applications is due on (1) week after the receipt of this order, and thereafter on the same day of each week, until Obligor finds and reports employment to the WCCSEA. Failure to file written proof of job applications or to report employment is punishable as contempt of court. IT IS FURTHER ORDERED that this ORDER shall remain in full force and effect during any present or future periods of unemployment of the OBLIGOR. IT IS FURTHER ORDERED that OBLIGOR notify the Washington County Child Support Enforcement Agency (WCCSEA) IN WRITING upon commencement of employment, including self-employment, upon the receipt of workers' compensation payments, upon the receipt of any other source of income, upon the opening of an account in a financial institution, or upon the occurrence of any of the events listed on the notification page attached to this order, by returning said page to the WCCSEA, and to include in the notification a description of the nature of any employment and the name and business address of any employer. IT IS FURTHER ORDERED that OBLIGEE immediately notify this Court IN WRITING of any reason why the support order should terminate, by returning the notification page of Obligee's copy of this order to the Washington County Child Support Enforcement Agency. IT IS FURTHER ORDERED that both the OBLIGOR and OBLIGEE are required to notify the Washington County Child Support Enforcement Agency IN WRITING of their current mailing address, residence address, and any change in either address, until further notice from this Court. WILLFUL FAILURE TO SUPPLY THE ABOVE INFORMATION IS CONTEMPT OF COURT. A copy of this order shall be served on the Obligee by certified mail, return receipt requested. If Obligor is not present to acknowledge receipt of the support order and the 3113.21 order, Obligor shall be served by certified mail, return receipt requested. This order shall remain in full force and effect until further order of this Court.
