Policy 4 - Domestic Relations Forms The following Forms shall be used in Domestic Relations Practice in Washington County, Ohio. (See appendix A for copies of Forms) Form 1 Court Information Sheet Copies for Court use - original plus 2 When Used - filed in all new cases and in all post decree Motions. Form 2 Financial Affidavit and IVD Application Copies for Court Use - completed original for party filing plus 1 copy for Service on Opposing Party, and blank original for Service on and Completion by opposing party. When used - filed in all new cases and post decree motions seeking modification of spousal or child support or modification of custody. Form 3 Addendum Entry - General Provisions Copies for Court Use - completed original plus 3 copies When used - filed with all child support and spousal support notices Form 4, 5, 6 Seek Work, Bond, Notice Only Orders Copies for Court use - Original plus 3 copies Form 7, 8 Notice to Payor/OBES/or Financial Institution to withhold Obligor Income/Assets, Addendum Entry - General Provisions Copies for Court Use - Original plus 4 copies When Used - required for all withholding orders. OBES order only used for IVD Child Support, not spousal support or non-IVD child support. Form 9 Notification Forms - Obligee, Obligor, Payor, Distributor of income, Financial Institution Copies - original plus 3 Obligor and Obligee Notifications. Original plus 4 of applicable notification form for withholder of income, assets. When used - Obligee, Obligor in all cases. Use forms appropriate for nature of withholding institution. Forms 10-13 Insurance Forms The forms used for Ohio Revised Code Section 3113.217 health insurance orders are as follows:Obligor Insurance Exclusive of copies for counsel, the number of copies required are as follows:Obligor Insurance -Original plus 4 Washington County Clerk of Courts
