Rule 26 Domestic Relation Forms

The Court from time to time approves and modifies forms to be used in domestic relations practice in this Court. These forms must be used and are required to be filed in each domestic relations action. These forms include:

Court Information Sheet with IV D application (See Appendix)
Affidavit of Income and Expenses (See Appendix)
Addendum Entry - General Provisions (See Appendix)
Ohio Revised Code Section 3113.21 Notices to Withhold
Obligor Income/Assets and Withholding Notice to Parties
Ohio Revised Code Section 3113.217 Insurance Forms.

Those forms not required by Statute, Administrative Code, or Supreme Court Rules are set forth the Appendix to the Rules of the Court and are available from the Washington County Child Support Enforcement Office and/or the Washington County Clerk of Courts Office.

DOMESTIC VIOLENCE FORMS

As required by Ohio Superintendence Rule 10, the Office of the Office of the Clerk of Courts shall distribute, upon request, packets containing forms and instruction sheets for the filling of Domestic Violence complaints, and temporary and final protection orders. The required forms or substantially similar forms shall be used in all Domestic Violence filings.
