Rule 27 Required documents if filing Dissolution, Agreed Modification of Child Support, Spousal Support, or Designation of Residential Parent and Legal Custodian

In addition to filing those documents required by law or rule, at the time of filing of a dissolution of marriage, agreed modification of residential parent designation, agreed modification of child support, or agreed modification of spousal support the following must also be filed:

  1. Information sheet, including signed application for IV-D Services, if residential parent designation or support for minor children is involved.
  2. Completed affidavits of income and expense for each party.
  3. Completed child support work sheet, if there are minor children.
  4. Proposed final order and all required copies.
  5. Proposed Withholding Notice and Notice to parties and all required copies.
  6. Proposed insurance order and all required copies, if applicable.
  7. Proposed Addendum Entry - General Provisions and all required copies
