FILING INSTRUCTIONS FOR THE "APPLICATION FOR PERMIT"
NOTE! STRUCTURE PLANS should include the following: drawn to scale showing the nature, dimensions, and elevations of the area in question; existing or proposed structures, fill storage of materials, drainage facilities; and the location of the foregoing. Specifically, the following information is required; (1) MSL (mean sea level) elevation of the lowest floor (including basements) of all structures; (2) MSL elevation to which any structure is floodproofed; (3) certification by a registered Professional Engineer or Architect that the floodproofing methods meet the community floodproofing criteria; and (4) a description of the extent to which any watercourse will be altered or relocated. Based upon the information provided, the administrator will determine if the project site is located in an identified Floodplain. If it is not, a letter will be sent to the applicant indicating that the project is not located in a Flood Hazard Area. No permit is required and the $10.00 filing fee will be returned. If the project is determined to be in an identified Floodplain, the administrator will determine the 100-year BASE FLOOD ELEVATION (lowest allowable floor elevation) and notify the applicant. Upon completion of the project, a registered Surveyor (State of Ohio) shall determine the exact lowest floor elevation and complete the 'ELEVATION CERTIFICATE FORM". The completed form shall be returned to the administrator and a letter of compliance issued. If an application for a permit to develop in a Flood Hazard Area is submitted after work has commenced, a penalty as set forth in section 3.7 of the FLOOD DAMAGE PREVENTION REGULATIONS shall be levied.
