The Children Medical Handicaps (CMH) program in the Ohio Department of Health through your County Health Department, Public Health Nurse. The goal is to help families with children with chronic medical conditions improve their child’s health outcome and quality of life. This program is available to children from birth to age 21. CMH helps families receive services from CMH-approved providers for diagnosis, treatment and services related to your child’s eligible medical condition. A public health/CMH nurse provides home visits, coordination with providers, services and community agencies. Application is through a CMH-approved doctor or nurse practitioner or your county public health nurse.
