There are several scenarios in which a voter may cast a provisional ballot. A provisional ballot may be used on election Day if a voter's eligibility is in question, or before (or on) Election Day if a voter has recently changed his or her address or name and did not update his or her voter registration.
The following is the policy and procedure for Public Records Requests:
1. When Public Records Requests are received at the Washington County Board of Elections they need to be time and date stamped. All requests must be prepaid if the requestor is not at the counter to pay for them as they are filled.
2. The following charges apply to Public Records Requests:

Fax if long distance $0.05 per page plus cost of copies prepaid, toll free number is free plus cost of copies prepaid
Email no charge
Copier $0.05 per page, if double sided $0.10 ea. Printer $0.05 per page, if double sided $0.10 ea.
Mail $0.06 for BOE envelope, $0.15 for legal size brown envelope plus postage, all of which should be prepaid
Labels $0.25 per page
3. For verbal requests, our board has adopted a policy that all requests must be recorded so the intent of the request is clear. The board will then have a record of the receipt time and date.
4. The public records requests will be filled in a timely manner. We ask for cooperation during periods near an election.

(Note: No voted ballot may be returned to a board of elections by fax or e-mail. If a voted ballot is returned by fax or e-mail, it will not be accepted, processed, or counted.)
In addition, the 17-year old electors must satisfy Ohio's voter identification requirements inaccordance with R.C.3505.18.
